Whitening is the most requested cosmetic dental service we offer. It's also the one with the most misinformation online. Let's clear it up.
How All Whitening Works
Every whitening product, professional or over-the-counter, uses hydrogen peroxide or carbamide peroxide as the active ingredient. The peroxide penetrates enamel and breaks apart the chemical bonds of stain molecules. That's it. The differences between products are concentration, delivery method, and time.
Zoom! In-Office Whitening
This is what we use at Sky Dental. Zoom! applies a high-concentration peroxide gel (25-38%) to the teeth, then activates it with a specialized light that accelerates the bleaching process.
Result: 6-10 shades lighter in a single 90-minute session.
Who it's for: Anyone who wants maximum results quickly. Patients with a wedding, event, or reunion coming up. Patients who've tried take-home and want a dramatic jump. Patients who just want the best possible result in one visit.
Caveat: Increased sensitivity for 24-48 hours afterward is common. We apply a desensitizing treatment at the end of the session, and most patients find it manageable.
Professional Take-Home Whitening
Custom-fitted trays are made from an impression of your teeth. You fill them with a lower-concentration prescription gel (10-22% carbamide peroxide) and wear them at home, typically 1-2 hours daily, or overnight, for 2-4 weeks.
Result: Similar final shade to Zoom!, achieved more gradually.
Who it's for: Patients who want professional-grade results without the in-office appointment. Patients with sensitivity who do better with lower concentration over more time. Patients who want flexibility in their schedule. Also ideal for maintenance after Zoom!.
Advantage over Zoom!: The final result can actually match or slightly exceed in-office whitening because you're treating for longer cumulative time. The trays are also yours to keep for future touch-ups with prescription refill gel.
Over-the-Counter Products
Whitening strips (Crest 3D Whitestrips, etc.) use hydrogen peroxide at concentrations between 6-10%. They work, just more slowly and to a lesser degree.
Result: 2-4 shades lighter over 2-4 weeks.
Who they're for: Patients with mild staining who want gradual improvement. Budget-conscious patients. Patients maintaining a result from a professional treatment.
Honest assessment: For patients with yellow staining from coffee, tea, or wine, strips can produce a satisfying result. They won't touch gray or blue-gray staining (often from tetracycline or fluorosis), which requires veneers rather than bleaching.
What Whitening Can't Do
Bleaching doesn't work on restorations, crowns, veneers, bonding, and fillings don't bleach. If you have visible restorations and want to whiten, you need to factor in that the restoration may need to be replaced to match the new shade.
Intrinsic staining (staining inside the tooth from tetracycline, excessive fluoride during development, or trauma) doesn't respond to peroxide. These cases usually require veneers.
Which One?
For most patients wanting significant, fast results: Zoom! followed by a take-home tray for maintenance.
For patients wanting a gradual improvement on a budget: take-home trays.
For mild staining or maintenance: OTC strips are perfectly fine.
